The Reality

The system isn't designed
to protect your loved one.

Hospitals are under enormous financial pressure to discharge patients as quickly as possible. Without someone in the room who knows the system — and knows your rights — families get left behind.

Premature Discharge

Hospital case managers carry 30+ patients at once. The phone call comes — "your mother is being discharged tomorrow" — with no real plan and no mention of your right to appeal. We stop this.

Inappropriate Placement

The path of least resistance is sending your loved one to whatever facility has an open bed — not the best one, not the right one. We ensure placement is based on clinical need, not convenience.

Undertreated Rehabilitation

Medicare covers up to 100 days of skilled rehab after a qualifying hospital stay. Most patients are discharged in 2–3 weeks because facilities benefit financially from freeing the bed. We push back.

Patients have the legal right to formally appeal a discharge they believe is premature. Fewer than 1% ever exercise this right — simply because no one told them it exists. We know it. We use it.

Phase 1 — From Day One

Hospital Patient Advocacy

We are physically present at the hospital — in care conferences, at the bedside, in conversations with physicians and case managers. We ensure the medical plan is appropriate and that no decision is made without your family's informed understanding.

  • Attend physician rounds and care conferences on the family's behalf
  • Review the clinical plan and ask questions families don't know to ask
  • Identify and challenge inappropriate or premature discharge decisions
  • File formal Medicare discharge appeals when warranted

Phase 2 — Post-Acute

Rehabilitation & Skilled Nursing Oversight

When your loved one transfers to a rehab or skilled nursing facility, our oversight continues. We visit regularly, verify that ordered therapy is being delivered, and advocate for the full rehabilitation benefit they are entitled to under Medicare.

  • Regular in-person visits to the facility
  • Verify therapy minutes are actually being delivered as ordered
  • Monitor for clinical decline, neglect, or early discharge pressure
  • Advocate for extended stay when clinically appropriate
  • Keep family fully informed with written updates

Phase 3 — Transition

Discharge Planning & Care Transition

When the time comes to go home — or to a longer-term setting — we ensure every piece is in place before the patient leaves the building. Not after they're already home and something goes wrong.

  • Complete medication reconciliation and written medication instructions
  • Home health agency coordination and confirmed setup
  • Durable medical equipment delivery confirmed before discharge
  • All follow-up appointments scheduled prior to leaving the facility
  • Family caregiver education and home safety review

Safe Landing — Home or Appropriate Placement

The goal is always the most appropriate outcome — whether that's safely home with support, or in a facility that genuinely matches your loved one's clinical, social, and personal needs. When placement is the right answer, we guide the search with clinical knowledge — not financial incentives.

  • If going home — full care plan handoff, family education, follow-up checks
  • If placement needed — facilities vetted based on need, not who pays us
  • Optional ongoing monthly retainer for continued oversight and peace of mind

The Right to Appeal a Discharge

Medicare patients have the legal right to formally appeal a discharge they believe is premature. The BFCC-QIO process delays discharge while the case is reviewed. We file this on your behalf.

Up to 100 Days of Rehab

Medicare covers up to 100 days of skilled nursing and rehabilitation after a qualifying hospital stay. Most patients never see day 20. We fight for every day your loved one is clinically entitled to.

The Right to Be Heard

Every patient has the right to participate in their own care plan and discharge decisions. We ensure that right is exercised — even when the system is moving too fast for families to keep up.
